It is important for ALL members, regardless of bargaining unit, to complete the necessary forms and submit to your administrator. I have included some information below for you to review so that you have a better understanding of why this is so important. Boards cannot collect, analyze and report data if the employees do not fill out the required forms.
REQUIREMENTS FOR BOARDS
Reporting of Violent Incidents
Boards are required to report the total number of violent incidents on an annual basis to the Ministry of Education through the Ontario School Information System (OnSIS). All violent incidents that occur on school premises during school-run programs must be reported to the ministry, whether the violent incident was committed by a student of the school or whether it was committed by any other person.
Ongoing Monitoring by Boards
Boards are also required to collect and analyze data on the nature of violent incidents to support the development of board policies and to inform the board and school improvement plans.

Provision of Information for Occasional Staff
The Provision of Information for Occasional Staff forms are intended to provide staff who are not regularly assigned to a particular school with critical and timely information in order to support student and staff safety. Key information includes key contact information for administration, safety and emergency response information, and unique classroom information they need to be aware of.
At the recommendation of the Joint Health and Safety Violence sub-committee and the support of Senior Management, we are pleased to provide revised classroom information sheets for staff for:
- Elementary Occasional Teachers
- Secondary Occasional Teachers
- EA/CYW Occasional & Extended Day Staff
- DECE Occasional & Extended Day Staff
- THR Secretarial Replacement Staff
- Library Clerk Replacement Staff
If you are a Permanent or LTO DECE, your administrator should have requested that you complete the DECE Occasional & Extended Day Staff forms and leave them with your Supply Notes.

WRDSB Toileting/Diapering in FDK
A new memo was put out by the WRDSB in January of 2019 and it is what DECEs now have to follow until we can bargain better language for the DECEs. It is advised that you follow your administrators' requests and keep a record/documentation of how much time you are spending in the bathroom supporting students' needs during your working day. Please remember to "protect" yourself and never be alone in a bathroom with a student with the door closed, if able have a second adult present or leave the door open. Level D EAs will not be performing toileting support in the kindergarten classrooms, unless they are assigned to a student that requires assistance and is supported with a medical diagnosis and documentation. The #5 excerpt from the 19-20 FAQs may assist in additional clarification for your reference.
